| Comments: | Seals were given until May 1, 2016 to provide signed Suspension Verification Forms for the games Ludwig Johnson sat out. Failure to provide such forms will result in a suspension of the Seals coach and Ludwig Johnson. As those forms were not provided, Seals forfeit this game 3-0 for playng a suspended player. Ludwig Johnson is suspended until Suspension Verification Forms are provided to the PAD Committee. The coach of the Seals team, Blagovest Tranchev is suspended indefinitely for playing a suspended player.
The PAD Committee does not review red card decisions made by referees. The suspensions for those red cards stand.
